How Home

Visits Work

Home visits are scheduled on a as needed basis.  At the time you call to schedule an initial appointment, a general health care questionnaire will be mailed to your home for you to complete. At the time of the first appointment, that questionnaire will be collected, as well as insurance information and any pertinent additional medical history you provide.

 

We will then conduct an initial visit to ensure that our practice can meet your medical needs. After establishing care, our nurse practitioners,  nurses and supportive staff will be coordinating and conducting home care visits in close collaboration with one another.

How to Make

An Appointment

To make an appointment for a home care visit, please call our office and ask to speak with Jessica Alvarez-Cruz, our Home Visit Coordinator.

You will be asked to provide some basic information:

  • patient's name

  • date of birth

  • home address

  • contact information

  • insurance information

  • current Primary Care Physician

  • some basic medical history

Call the office if you wish to schedule a home visit

8:30 am to 5:00 pm

24/7 for emergent issues
