As participants in Medicare's value based " Primary Care First " model  we would like for you to receive the full benefit as a participating member and ask that you please align voluntarily as a Primary Care First patient by following these steps : 

 

1. Go to https://go.cms.gov/3h2KHWl

2. Enter  Oradell or the zip code 07649 under " location"

3. Choose " Family Medicine " as the specialty and select Dr. Hoinkis from the list of providers

4. Log in or create account on Medicare.gov

Participation will not impact your financial responsibilities or care - in fact it will give us access to all tools provided by Medicare to better recognize and coordinate your needs .
